How tourniquets actually work
And three ways you are probably using one wrong.

the mechanism
It is a pressure argument, not a knot.
A tourniquet does one thing: it presses on a limb hard enough to flatten the artery running through it. Arteries carry blood at pressure. To stop flow, the pressure you apply from outside has to exceed the pressure inside — and on a thigh, wrapped in muscle, that is a great deal more force than most people expect. Everything that goes wrong with tourniquets goes wrong at that one number.
This matters because of what happens when you fall short. Veins are low-pressure vessels; they collapse long before arteries do. A tourniquet that is tight but not tight enough shuts the outbound veins while the inbound artery keeps pumping. The limb fills. The bleeding gets worse. A half-applied tourniquet is not half a tourniquet — it is a mistake with a direction, and the direction is against you.
Two design features exist to get you across that pressure line. The first is width: a broad strap spreads force over more tissue and occludes at a lower applied load, which is why a real tourniquet strap is an inch and a half of webbing and not a shoelace. The second is mechanical advantage — the windlass you twist, or the ratchet you crank. Hand strength alone almost never reaches occlusion pressure on an adult leg. That is the honest reason improvised tourniquets so often fail: not bad luck, and not bad technique, but narrow material with no way to multiply force. A belt looks like a tourniquet and is not one.

Each of these is common, each is taught wrong somewhere, and each turns a working device into a decoration.

Stopping when the bleeding slows
Slowing is not the endpoint. Bleeding stopped is the endpoint, and the check that confirms it is the pulse below the tourniquet: it should be gone. If blood still wells up, the device is functioning as a venous tourniquet and is actively making things worse. Tighten it further. If it will not go further, place a second tourniquet directly beside the first, just above it, and tighten that one too. Two tourniquets on one leg is a normal, taught answer — not an admission of failure.

Placing it carefully instead of fast
Civilian courses teach two to three inches above the wound, never over a joint, and that is the right target when you can see what you are doing. But an arterial bleed does not wait while you cut away a trouser leg to find the exact source. If the bleeding is obviously life-threatening and the source is not obvious, go high and tight on the limb, over the clothing, and get it stopped. You can place a second, better-sited tourniquet later, once nobody is dying in front of you.
The joint rule holds either way. Over a knee or an elbow there is nothing solid to compress the artery against, so the pressure goes nowhere. Move above the joint.

Loosening it to “let some blood back in”
Once a tourniquet is on and working, it stays on until someone with a hospital behind them takes it off. The old instruction to release it periodically is obsolete and dangerous: each release restarts the bleeding you just stopped, and costs blood the patient does not have to spend twice.
Write the time of application on the device, or on the skin above it, and say it out loud at handover. Limbs tolerate this for hours, not minutes. A limb at risk is a problem for a surgeon with time; a bleed you reopened on a roadside is a problem for nobody, afterwards.

A real device
Buy a windlass or ratchet tourniquet from the current CoTCCC-recommended list — the CAT, the SOF-T Wide and the SAM XT are the familiar names — and buy it from the manufacturer or a known medical supplier. Counterfeits of all three are everywhere on general marketplaces, they are the same shape and they break under load.

Where you can reach it
Carried on your person or in a fixed, known place — not at the bottom of a bag in the boot. Assume you are reaching for it with one hand, because the injury is often yours.

One-handed, on yourself
The rep that matters is applying it to your own thigh, one-handed, in under a minute. Buy a second one to train with. The one you train with never goes back in the kit.

On the device, on the skin, in a photo on your phone — anywhere the next person will find it.

The expectation of pain
A correctly applied tourniquet hurts badly, and a conscious patient will ask you to loosen it. That pain is the sound of the device working. It is not a reason to loosen it, and knowing that in advance is most of the battle.
the limits
What a tourniquet cannot do.
It only works where there is a limb to squeeze and bone to squeeze against. The groin, the armpit, the neck, the torso — the junctional bleeds — give it nothing to work with, and they are not rare. For those, the answer is packing the wound and holding hard, directly, for longer than feels reasonable. That is a separate skill and its own draw.
It also does not end the emergency. It buys the minutes in which everything else — the call, the transport, the surgeon — has to happen. A tourniquet is the reason someone is still alive when help arrives. It is not the help.

Take your training tourniquet. Sit on the floor. Put it on your own upper thigh, one-handed, with your eyes shut, and turn the windlass until you cannot turn it again. Note what that took — the force, the seconds, the noise you made. That number is the only part of this page your hands will remember under stress.
